Live Well at Home Care Coordinator

Job Responsibilities:

Responsible to screen and assess frail older adults; develop an action plan to reduce risk, provide community supports, and provide home aids and minor/major home repairs to improve the physical safety of the elder’s home.

Job Qualifications:

Bachelor’s degree in counseling, sociology, psychology, or related field, or a combination of education and experience.  Experience working with older adults and knowledge of community agencies, programs and resources. The position involves regularly traveling the service area and requires a valid driver’s license in addition to being computer literate.

Needed Attributes: Ability to communicate effectively, both verbally and in writing to the media, the public and community members at all income levels. Works effectively with teams and independently. Ability to manage multiple priorities. 

Essential Functions:
  • Conduct personal health screenings and home safety assessments
  • Develop an action plan to reduce risk, provide supports and referrals to appropriate resources
  • Coordinate with AEOA Housing Services for minor/major home repairs to improve physical safety
  • Provide follow-up information and support to clients as needed
  • Network with referral sources to increase awareness of services and encourage referrals
  • Represent the agency through outreach efforts and public speaking engagements as required
  • Participate in professional development in-services and trainings as appropriate
  • Maintain appropriate record keeping per funding requirements

Temporary FT Live Well at Home Care Coordinator position open in the Virginia Senior Services office. M-F, 31 hrs/week. Starting wage: $16.37/hr. Please apply by November 26, 2019.
