Responsible to screen and assess frail older adults; develop an action plan to reduce risk, provide community supports, and provide home aids and minor/major home repairs to improve the physical safety of the elder’s home.
Bachelor’s degree in counseling, sociology, psychology, or related field, or a combination of education and experience. Experience working with older adults and knowledge of community agencies, programs and resources. The position involves regularly traveling the service area and requires a valid driver’s license in addition to being computer literate.
